What Are Some Possible Causes of Shrinking Height?

1. Age-Related Shrinkage

Most people tend to lose some height around age 50, with the decline often accelerating in later decades. In fact, studies indicate that on average, individuals may experience a 2cm to 4cm reduction in height over the course of their life1. This is often caused by declining muscle mass and bone density.

2. Sarcopenia (Muscle Loss)

Sarcopenia is the age-related loss of muscle mass and strength. When the core and back muscles weaken, it becomes harder to keep the spine upright. This may cause a stooped posture, reduced balance, and faster fatigue when standing or walking.

3. Osteoporosis and Vertebral Fractures

Osteoporosis weakens the bones and increases the risk of vertebral compression fractures, where part of a vertebra collapses and reduces spinal height. Over time, one or more of these fractures can lead to noticeable height loss. In one study, people with a vertebral fracture had, on average, 0.54 inches more height loss than those without a fracture2.

4. Spinal Deformities

Spinal deformities in adults, such as kyphosis and scoliosis, may also reduce standing height by changing the natural alignment of the spine3. Kyphosis causes excessive forward rounding, while scoliosis causes a sideways curve. In more severe cases, these conditions may also lead to pain, muscle fatigue, poor balance, or difficulty standing upright.

5. Spinal Disc Dehydration

Spinal discs sit between the vertebrae and act as shock absorbers. Their high water content helps them stay flexible and maintain their height. During the day, pressure from standing, sitting and movement causes the discs to lose some fluid, which they partly regain during rest. This is why some individuals may be slightly taller in the morning.

With ageing or disc degeneration, the discs may retain less water and become permanently thinner. Across several levels, this can contribute to gradual height loss, stiffness, or reduced flexibility.

When Should You Be Concerned About Shrinking Height?

Shrinking height should be assessed when it is noticeable, progressive, or occurs over a short period.

See a doctor if it is accompanied by:

  • Persistent or sudden back pain
  • A more stooped posture
  • Difficulty standing upright

What You Can Do About Spine-Related Height Loss

Treatment depends on whether shrinking height is caused by a vertebral fracture, spinal deformity, or another underlying condition. Lost height cannot always be restored, but treatment may help reduce pain, prevent further progression, and preserve mobility.

For a Vertebral Fracture

The approach may differ slightly depending on the location and the severity of the fracture. If it is stable, then the fracture may be managed with conservative methods, such as pain medication, short-term bracing, activity modification, and physical therapy.

If osteoporosis is a contributing factor, your doctor may also recommend bone-strengthening medication along with calcium and vitamin D assessments. Fall-prevention measures may be advised to reduce the risk of further injury.

If pain persists or the fracture causes ongoing instability, stabilisation surgery may be recommended. For example, a lumbar spine fracture​ may be treated with minimally invasive procedures such as vertebroplasty or kyphoplasty. These procedures stabilise the fractured vertebra by injecting medical bone cement, helping to support the bone while minimising disruption to the surrounding muscles and soft tissues. 

In more complex or unstable fractures, spinal instrumentation with screws and rods may be needed to restore stability.

If Height Loss Is Caused by a Spinal Deformity

In adults, mild spinal deformities may be managed with physical therapy, posture training, strength exercises and regular monitoring. These can help improve balance, reduce muscle fatigue, and help you remain more upright, although they may not reverse an established curve.

If the deformity continues to progress, causes persistent pain or compresses nearby nerves, surgery may be considered. For example, if a spinal deformity leads to nerve compression, minimally invasive or endoscopic decompression surgery may be performed to relieve pressure on the affected nerves while minimising tissue disruption. 

When the main concern is worsening alignment, curve progression or loss of spinal stability,  corrective spinal fusion surgery with rods and screws may be recommended. Your doctor will consider your symptoms, imaging findings, and overall spinal balance before recommending the most appropriate approach.
